SDFI®-TeleMedicine is pleased to introduce a FREE SDFI®eForm designed solely for SART, SANE & SAFE programs.

SDFI®eForms provide a free and easy way to type into an eForm, print an e-Form, save your electronic form as a PDF file, recall your eForm and make changes to your eForm field data.   (PDF files are 100% compatible with the SDFI®-TeleMedicine process and system).

To export and analyze data, you will need to purchase additional software. You can get the required "off-the-shelf" software from many locations.

You can use our SDFI® eForms with Adobe's FREE Acrobat Reader right now.  Later, you can upgrade your software tools from a wide variety of retailers. When you do, you will be able to export your SDFI® eForms data.  This means that you can use SDFI® eForms today and later export your SDFI® eForm data when or if you choose to purchase and install Adobe Acrobat Pro and MS Excel 2007.  We do not sell or support Adobe Acrobat Pro and MS Excel.   (NOTE:  Microsoft Excel 2003 will not work.)

Licence Agreement

License Agreement on the use of SDFI®-TeleMedicine eForms.    

This is a license agreement between you, a final user of computer software, and SDFI®-TeleMedicine LLC (hereinafter "SDFI®" or "SDFI®-TeleMedicine"). Please read it carefully!  By downloading the eForm software, you express your approval of the conditions and terms hereof and you are bound thereby. 

Should you disagree with the conditions and terms hereof, promptly terminate the use of this software and computer code and destroy any copies thereof and/or delete the software and computer code already downloaded.

1. Providing the License.  SDFI®-TeleMedicine through this License Agreement grants to you non-exclusive rights.  You may install (download) and use multiple copies of the software, provided that you abide by this license agreement at all times. You may use the software in a network or on more than one computer. As this is a “FREE” version of the software it is not subject to any guarantees or warranties. Support is NOT provided.  Use the eForm at your own risk. 

2. Copyrights. The computer software and computer code subject thereto is and continues to be a property of the SDFI®-TeleMedicine. The computer software and computer code is also subject to copyrights and as such it is subject to a full protection thereof.

3. Use Restrictions. You must not use the software or computer code in any manner that might somehow jeopardize or violate the manufacturers' copyrights. In addition, you must not reproduce the software, change it, modify it, decompile it, transfer it from a mechanical code, reanalyze it or create any deduced works from the software. All rights are reserved. Any copy of the software you have been permitted to obtain based hereon must contain a complete copyright and other notices provided at the original duplicate.

4. Assignment Restriction. This license is provided personally to you and for that reason it does not allow you to make any duplicate (copy) to be sold, borrowed, assigned, leased or transferred in any manner whatsoever to another person or company. Any transfer executed in violation of this provision shall be deemed invalid and constitutes a reason for termination of your license validity.

5. Liability Limitation. UNDER NO CIRCUMSTANCES WILL SDFI®-TELEMEDICINE BE LIABLE TO YOU, OR ANY OTHER PERSON OR ENTITY, FOR ANY LOSS OF USE, REVENUE OR PROFIT, LOST OR DAMAGED DATA, OR OTHER COMMERCIAL OR ECONOMIC LOSS OR FOR ANY DIRECT, INDIRECT, INCIDENTAL, SPECIAL, STATUTORY, PUNITIVE, EXEMPLARY OR CONSEQUENTIAL DAMAGES WHATSOEVER RELATED TO YOUR USE, EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES OR IF SUCH DAMAGES ARE FORESEEABLE. THIS LIMITATION SHALL APPLY EVEN IN THE EVENT OF A FUNDAMENTAL OR MATERIAL BREACH OR A BREACH OF THE FUNDAMENTAL OR MATERIAL TERMS OF THIS AGREEMENT.

A Note to Forensic Nurses Based in California

Over the last five years, there have been discussions about the use of our SDFI electronic form in place of the paper based CalEMA 2-923:  Acute Adult/Adolescent Sexual Assault Exam Form.   ( http://www.ccfmtc.org/forensic.asp )

Other than formatting, layout and style, the two forms are effectively the same and serve the same purpose.   The same data, plus, is collected and used by SART members around the state and around the country.  The SDFI form has also been used and accepted by related levels of government.  This specifically includes the California state legal system.

Using an electronic version of a form that has been converted and updated from a paper format supports the evolving advancement in technology and DOES NOT conflict with California state law.

The use of electronic forms supports and improves medical examinations and evidence collection. By design, electronic forms support quality medical examinations, existing preservation procedures, law enforcement investigation and case prosecution.  Electronic forms are also consistent with the needs of law enforcement and crime laboratory policies.

There are a number of companies that sell "PAID FOR" electronic forms that are accepted by the California legal system, just as SDFI's "FREE FORM" is accepted by the California legal system.  The difference between paper based forms and electronic based forms is flexibility, formatting, layout and style.  Electronic forms have it, paper based forms do not.  

Why would you NOT use electronic forms?


THE CALIFORNIA GOVERNMENT SUPPORTS THE USE OF ADVANCING TECHNOLOGY

The State of California specifically supports the use of advancing technology.   In the introduction section of the CalEMA protocol itself, in the last paragraph of PDF page 13, you will find the following "printed" statement:


C. STATUTORY STANDARDS AND THE PROTOCOL

"The protocol, including forms and instructions, contains recommended methods for meeting the statutory standards. Flexibility is needed, however, to accomplish these tasks as circumstances may vary and new methods may evolve with advancing technology and research.

The important considerations are whether alternate methods support quality medical examinations, evidence collection and preservation procedures, law enforcement investigation, and case prosecution; and whether these methods are consistent with local law enforcement and crime laboratory policies."
